Coronado Veterinary Hospital offers our patient form(s) online so you can complete it in the convenience of your own home or office.

  1. If you do not already have AdobeReader® installed on your computer, Click Here to download.
  2. Download the necessary form(s).
  3. You can then fill them out online print it and fill out with pen.
  4. Fax to 619-435-7734, email to [email protected], or bring it with you to your appointment.

New Patient Registration Form - Required. This let's us know important information about you and your pet so that we can effectively and efficiently provide the service you deserve.


Drop-Off Appointment Form - current clients only. For appointments where the Veterinarian has requested your pet be dropped-off for an appointment. For current clients/patients only.


Boarding Form Canine - required for every dog boarding at check-in.

Boarding Form Feline - required for every cat boarding at check-in.


Anesthetic Procedure Consent Form - required for all procedures requiring general anesthesia

Veterinary Treatment Authorization by Owner for Caretaker - required for the treatment of any pet in the absence of the owner. Often used when owner is on vacation/working out of town for extended period of time. Must be filled out and signed by legal owner.


Heartworm Test Waiver - We require all canine patients to be tested for heartworm every year regardless of their prevention treatment. If you do not wish to obide by this protocol we require a waiver to be signed, ensuring you understand the dangers in involved with waiving heartworm testing.


Checklist for Traveling with Pets - required for pets who need official health certificates for travel. Fill this checklist out entirely and fax, email, or give to your Veterinarian prior to scheduling travel.
